1168 Case Report: Lower Limb Trauma with Vascular and Orthopaedic Injury in a National Level Gymnast

Abstract Traumatic injuries in high-functioning athletes can have devastating consequences and require cross-disciplinary management for both initial presentation subsequent management. We would like to present the case of a 17-year-old female, national level gymnast who presented with traumatic lower limb injury. During run-up trampoline, her right knee gave way immediate pain, deformity, inability weight-bear. At assessment she was noted large joint effusion straight leg raise Xray showed tibial plateau avulsion fracture. Initially neuro-vascularly intact, however on splinting, posterior dislocation occurred loss foot pulses, sensation, motor function leg. Computerised tomography angiogram an 8cm filling defect at popliteal artery. A multidisciplinary approach between Vascular Orthopaedics decided surgical support from Paediatrics Anaesthesia. The artery approached posteriorly long-term durability function, despite potential difficulty exposure, medial bailout. Exposure revealed section torn intima occlusion which resected replaced long saphenous vein interposition graft. Finally, four-quadrant fasciotomies spanning external fixation performed. Completion diagnostic confirmed graft patency restored 2-vessel run off. patient recovered this limb-salvage procedure although required several more orthopaedic procedures, eventually discharged mobilising crutches. This illustrates importance managing complex trauma especially high-performing individuals.
